Position Overview
We are seeking experienced Per Diem Nurse Practitioners (NP) and Physician Assistants (PA) to conduct in-person, no-cost Annual Preventive Health Risk Assessments for Medicare, Medicaid, and Affordable Care Act (ACA) members enrolled in designated health plans.
Assessments are completed in the member’s personal living environment and are designed to support preventative care, risk identification, and overall member satisfaction, in alignment with health plan and quality expectations.
Update: Minimum of 15 completed visits per month required.
Responsibilities
Perform comprehensive, in-home preventive health risk assessments.
Ensure high-quality assessments that meet member satisfaction, health plan expectations, and quality standards.
Provide 15 or more appointment slots per month.
Complete and submit accurate chart documentation within 24 hours of each visit.
Resolve documentation or charting queries within 48 hours.
Maintain timely and professional communication with clinical leadership.
Participate in required self-paced and instructor-led training, meetings, and updates.
Comply with HIPAA standards and organizational policies.
Comfortably travel to and work within members’ personal living spaces.
Schedule & Compensation
Appointment Length: Flexible 60–75 minutes per visit
Pay: Market rate per completed assessment
Mileage Reimbursement: Paid after the first 30 miles per day at the current IRS rate
Required Qualifications
Education
Master’s or Doctorate degree from an accredited Nurse Practitioner program (FNP, AGNP, or ANP), or
Master’s degree from an accredited Physician Assistant program.
Experience
Home health experience required within the past 5 years.
1 year of in-home assessment experience strongly preferred.
Licensure & Certifications
Active, unrestricted RN and NP or PA license in the practicing state.
Board certification:
NP: ANCC or AANP
PA: NCCPA
Active Medicaid ID number (state-specific).
Active Medicare PTAN number.
Current BLS certification.
Valid, unrestricted state driver’s license.
Proof of adequate auto insurance coverage for assigned service areas.
Credentialing Requirement
Providers Must Be Credentialed (or Credentialable) With
Excellus
New York State Medicare
Medicaid
This is required to ensure no delays in onboarding or assignment readiness.
Core Competencies

Ability to make independent clinical decisions while following established protocols.
