Overview

The Clinical Intake Coordinator facilitates the home health and hospice referral process by receiving referrals from multiple referral sources within and outside the Saint Luke's Health System. The role performs initial evaluation of referrals for home health appropriateness under Medicare, Medicaid, and commercial insurance regulations, determines the need for additional information from referral sources, communicates agency acceptance or decline of referrals, and processes accepted referrals.
Work / Responsibilities

Facilitates the home health and hospice referrals.
Provides specialized support for the Patient Accounting and Finance departments.
Serves as liaison between the patient and facility/physician and the third-party payer.
Prepares and defends the level of care and medical necessity for the assigned case.
Collaborates with the physician advisor, payor representative and site case managers to facilitate appropriate level of care decisions and billing status, and ensures compliance with Utilization Review standards and regulations.
Occasionally covers to ensure compliance with home health face-to-face requirements and county reporting for hospice.

Qualifications / Job Requirements

Applicable Experience: 2 years
Family Care Safety Registry - Missouri Department of Health & Senior Services, Registered Nurse (MO) Multi-State - National Council of State Boards of Nursing (NURSYS)
Associate Degree - Nursing
