
Clinical Intake Coordinator - Registered Nurse

Job Summary
The Clinical Intake Coordinator facilitates the home health and hospice referral process by receiving referrals from multiple referral sources within and without the St. Luke's Health System, performing initial evaluation of referral for home health appropriateness under Medicare, Medicaid, and commercial insurance regulations, determining need for additional information from referral sources, communicating agency acceptance or decline of referral to varied referral sources, and processing accepted referrals.
Key Responsibilities
Facilitate home health and hospice referrals.
Provide specialized support for the Patient Accounting and Finance departments.
Serve as liaison between the patient, facility/physician, and third‑party payer.
Prepare and defend level of care and medical necessity for assigned case.
Collaborate with physician advisor, payer representative, and site case managers to facilitate appropriate level of care decisions and billing status, ensuring compliance with Utilization Review standards and regulations.
Occasionally cover to ensure compliance with home health face‑to‑face requirements and county reporting for hospice.
Qualifications & Requirements
Registered Nurse with Missouri (MO) and Kansas (KS) licensure.
2 years of applicable experience.
Family Care Safety Registry – Missouri Department of Health & Senior Services, Registered Nurse (MO) and Multi‑State – National Council of State Boards of Nursing (NURSYS) credentials.
Associate Degree in Nursing.
Experience in home health or hospice preferred.
Experience with referral acceptance and appeals preferred.
