Position Summary
The Case Manager is responsible for overseeing all aspects of client case coordination, including treatment planning, documentation, communication with stakeholders, and ensuring compliance with DFPS and licensing standards. This role works closely with direct care staff, therapists, schools, and external agencies to support positive outcomes for youth.
Key Responsibilities
Develop, implement, and monitor individualized service plans (ISP) and treatment goals
Maintain accurate and timely documentation, including progress notes, incident follow‑ups, and reports
Coordinate with DFPS, SSCC (e.g., EMPOWER/2INgage), therapists, schools, and medical providers
Schedule and facilitate treatment team meetings and staffing
Monitor client progress and adjust plans as needed
Ensure compliance with state regulations, licensing standards, and company policies
Advocate for clients’ needs across home, school, and community settings
Support discharge planning and transition services
Qualifications
Bachelor’s degree in Social Work, Psychology, Human Services, or related field (required)
Experience working in residential treatment, foster care, or behavioral health preferred
Knowledge of DFPS regulations and Texas RTC standards preferred
Strong documentation and organizational skills
Ability to manage multiple cases and deadlines effectively
Excellent communication and teamwork skills
Preferred Skills
Experience with trauma‑informed care and behavior management
Familiarity with IEPs, school coordination, and community resources
CPI or similar crisis intervention training (or willingness to obtain)
Work Environment
Residential treatment setting working with youth ages [insert age range].
Fast‑paced, team‑oriented environment.
Must be able to handle crisis situations with professionalism and composure.
