This position is a full‑time role working Monday‑Friday 8am‑5pm.

What You Will Do
The Home Care Clinical Manager is a professional nurse who oversees the daily clinical activities and needs for the Home Care patient’s care in their home environment (private residence, assisted living, hospital, etc.) based on individual needs. The Home Care Clinical Manager is responsible for the oversight and management of the Home Care census and potential referrals. The Home Care Clinical Manager serves the Home Care team by utilizing advanced assessment, teaching and decision‑making skills and professional and effective leadership skills, all while keeping the individual patient goals of care. The Home Care Clinical Manager is the liaison between the patient, caregivers, Home Care IDG members, providers, Medical Director, referral sources and community sources and serves as a resource for all Home Care regulations, policy and procedures, departmental and organizational goals and best practice. The Home Care Clinical Manager documents in the medical record in a thorough and timely manner according to agency requirements. The Home Care Clinical Manager oversees and facilitates Home Care Case Conference and team meetings. The Home Care Clinical Manager supervises and oversees the assigned Home Care staff by providing clear expectations, guidance and facilitating an empowered and engaged workforce. The Home Care Clinical Manager performs Administrative On‑Call responsibilities, Holiday staffing and assists with emergency preparedness needs.
Required Education
Bachelor’s Degree in Nursing.
Preferred Education
Master’s Degree in Nursing.
Required Skills
Nursing skills as defined by generally accepted standards of practice. Basic knowledge and skills to care for the clinical needs of the population served. Professional and efficient verbal and written communication skills. Good interpersonal skills. Ability to multi‑task, basic organizational and time‑management skills. Basic computer skills. Knowledge of general office and administrative practices and procedures. Knowledge of how to utilize office equipment (copy machine, fax machine, etc.). Ability to be open, sensitive, flexible, and ecumenical. Must have demonstrated experience in working with patients and caregivers dealing with acute and chronic illnesses. Must be capable of prioritizing and handling multiple tasks of critical importance simultaneously.
Preferred Skills
Home Care Home Base experience; Experience with smart phones and smart devices; Experience with Microsoft Office.
Required Licensure / Registration / Certification
Current Texas state license in appropriate discipline; Current American Heart Association CPR; Current Texas state driver’s license; Current auto insurance. Must have reliable transportation.
Preferred Licensure / Registration / Certification
OASIS certified; Wound Care certified; Advanced education or certification in area of specialty.
Required Experience
Two years experience in a healthcare setting in the respective discipline.
Preferred Experience
Medica‑Surgical experience; Experience in Case Management; Supervisory position in healthcare setting; Home Care of Hospice experience.
