Position Summary
The Case Manager will provide support and service linkages to consumers who reside within the Abbey Manor CR/SRO. Coordinate services and ensure quality care to the consumers. This position may have supervised/unsupervised contact and/or conversational interaction with consumers while working in the residences, day treatments and habilitation facilities.
Essential Accountabilities
Provide direct care services to consumer, including planning and implementation of goals and training, which will develop and enhance activities of daily living skills. These include but are not limited to:
Budgeting
Grooming
Nutritional planning
Use of public transportation/travel training
Use of community resources/community integration
Shopping/menu planning
Housekeeping
Sexual development/socialization skills
Physical health maintenance
Assertiveness/self advocacy training
Monitor consumer’s progress, performing manual tasks, reading and writing functions such as documentation, lifting, climbing and descending stairs, traveling, boarding and using public transportation (bus, subway), use of telephone to make and receive calls.
Coordinate evacuation and ensure consumers’ safety in emergency situations (e.g., fires, fire drills), including ability to exit in emergencies, climb and descend fire escapes, assist others.
Maintain all necessary written documentation, including functional assessments, medication updates, progress notes, quarterly service plans, completion of progress reports and other assigned paperwork.
Assist consumers in planning and coordinating recreational/social activities, escorting them as needed and participating in moderate physical activities.
Maintain and update consumer files monthly.
Attend area case management meetings.
Report and document incidents within 24 hours to immediate supervisor or Deputy Director of MHS.
Provide crisis intervention, follow‑up, and complete 30‑day follow‑ups on discharged consumers.
Transport self to various sites and attend meetings/training as required; count, add, subtract, and manage money.
Communicate verbally and in writing with staff and supervisors to give instructions, provide feedback, and adjust work practices.
Participate in required training such as CPR/First Aid, Medication Supervision, Cultural Competence, CPI, TIP‑42, Motivational Interviewing, and Suicide Assessment and Prevention.
Other Responsibilities
Participate in activities of other staff members in their absence or during staff shortages.
Represent the agency at meetings and trainings not otherwise specified.
Work flexible schedule; locations and schedule may change.
Drive facility vehicle when needed (escort, field trips, etc.).
Maintain confidentiality at all times.

Qualifications
A Bachelor’s Degree preferred with a minimum of 1 year working with homeless, mentally ill chemical abusers, or an Associate’s Degree and 2+ years working with special populations, or a High School Diploma and 3 years experience with special populations.
