The Work

The work you'll do is more than just a job. As a Health Facility Evaluator, you will use nursing knowledge to support surveillance and evaluation of health care providers to verify compliance with Federal and State laws and rules related to nursing and health services.
Responsibilities
Evaluate health care providers using nursing and health knowledge and survey document expertise.
Document conclusions to determine degree of regulation compliance.
Conduct periodic unannounced follow-up visits to health care providers to verify compliance with correction orders and deficiencies.
Attend legal proceedings or hearings regarding deficiencies, penalty assessments, abuse
eglect, or termination/adverse action situations.
Positions will be based in the St. Cloud office with 75% onsite surveillance of facilities in the South/South Central Minnesota region. When not onsite, follow-up work is performed by teleworking from home.
Additional Requirements
Candidates must be available for occasional weekend work. Overnight travel may be up to 75% of the time. Most work hours are 8:00 a.m. – 4:30 p.m., Monday – Friday; weekend or overnight travel requirements depend on inspection workload.
Reimbursement for authorized business travel is provided per bargaining unit guidelines.
Telework Policy

In accordance with HR/LR Policy #1422 Telework, this position is eligible for up to 50% telework for applicants living in Minnesota or a bordering state within 50 miles of the assigned work location. Applicants living outside Minnesota or in a bordering state within 50 miles must relocate to Minnesota within 30 days of start date. Policy details may exempt some employees from the 50% in-office requirement depending on job requirements.
Position Status

This vacancy may fill up to two Health Facility Evaluator roles. The department funds positions through grants and state dollars; future funding may affect staffing. The department will follow applicable collective bargaining agreements and compensation plans in the event of funding changes.
Minimum Qualifications

Currently licensed in Minnesota as a Registered Nurse, without restriction. Two (2) years of professional experience in a health care setting in roles such as RN, Charge Nurse (minimum two years as an RN Charge Nurse), RN Supervisor, Director of Nursing, or Health Care Administrator. A master's degree in nursing, Public Health, Health Care Administration, Psychology, Social Work, Medical Field or closely related area may substitute for 18 months of experience; a bachelor's degree may substitute for one year of experience.
Additional requirements to be assessed at interview:
Oral and written communication skills sufficient to interview staff, residents, clients and patients and to document concise, accurate information. Listening skills to ascertain compliance levels.
Critical thinking to collect and analyze information, identify focus areas, and make evidence-based decisions.
Interpersonal skills to develop and maintain professional relationships and participate in legal proceedings.
Computer skills to document findings and conclusions.
Decision-making skills to conclude surveys and defend legal actions related to nursing, medical and health care.
Preferred Qualifications

Successful completion of the Federal Surveyor Minimum Qualifications Test (SMQT).
Experience with the federal informal dispute resolution process for federally certified nursing homes.
Knowledge of federal and state laws and regulations related to nursing and health services and the ability to interpret them.
Experience conducting internal quality assurance activities.
Experience in federally certified facilities (e.g., nursing homes, supervised living facilities, etc.).
Additional Requirements

Successful completion of background and employment reference checks.
Travels without requiring a valid driver's license; travel requirements apply.
Completion of the Federal Surveyor Minimum Qualification Test (SMQT) after MDH-provided training.
