FLSA : Non-Exempt (Hourly)

Schedule : Part-Time; Three 10 hour shifts (10a-8pm) with every other Saturday rotation.

The Hoy Center at Westminster-Canterbury on Chesapeake Bay takes a holistic approach to skilled nursing care and aging. Our person-centric living aligns the delivery of care with individual needs and preferences. The Hoy features home‑like, comfortable settings, which we call Lighthouses, each with a family room and e‑concept kitchen. These modern and open private rooms are comfortable, well‑appointed and tastefully designed. The Lighthouses include the Rose Island, Mission Point, Point Lookout, Cape Henry and Point Roberts.

Residents on each floor enjoy breathtaking views and refreshing ocean breezes on three spacious and striking outdoor terraces, we call Observation Decks. The Fletcher Prayer Room, which provides a dedicated, peaceful sanctuary for contemplation and prayer is also available to our residents.

The Admissions Nurse
A critical member of the care management team for patients and aids in the assessment, planning, implementation, and evaluation of patient care and resources for both the admission and discharge of patients through various care levels. Responsible for monitoring the quality of care to ensure all aspects of services are appropriate, including infection control and risk management. Serves as a liaison between interdisciplinary team members to uphold continuity of care with patients transferring across care settings. The Admissions Nurse assists in admission planning, pre‑admission screening, and discharge planning. What You’ll Do In This Role

Work closely with the Admissions team and assist the Clinical team with the admission of patients including electronic medical record charting, nursing assessment and initiation of plan of care.

Support clinical nurses by coordinating admission activities, serving as a liaison to other team members and ensuring that all aspects of admission/discharge are complete for patients.

Assist in medication reconciliation upon admission to healthcare center and processing of medication orders with pharmacy.

Ensure admission orders and assessments are completed and communicated to appropriate team members in a timely manner.

Assess patient’s needs and work with patients/families to develop a baseline plan of care.

Support patient’s continuity of care by monitoring scheduling of PCP appointments, specialist appointments, and discharge education.

Assist in arrangement of follow up services, reinforce patient instructions and preparations for discharges as needed.

Maintain communication, documentation and reporting as outlined by the Director of Nursing and Healthcare Administrator.

Perform other duties as assigned; including stepping in to provide direct patient care on the floor as a Nurse during staffing crises.

Mobility required to climb or decline 5 flights of stairs.

Required
The Experience You’ll Bring

Active, unencumbered Registered Nursing License for the state of Virginia (or compact state).

At least one year of prior experience in Skilled Nursing centers.

Maintains a current knowledge of the legal aspects of nursing and of the policies, procedures and regulations.

Flu‑Vaccinated.

Preferred

Two or more years experience within Skilled Nursing or Long Term Care.

Experience with continuity of care.

COVID‑19 Vaccinated. As of now, the 2025-2026 COVID-19 vaccine formulation is being distributed to providers and pharmacies; however, it has not yet received official approval from the CDC. In Virginia, pharmacists may administer COVID-19 vaccines under the Virginia Board of Pharmacy’s statewide protocols or with a direct order from a licensed medical provider. We encourage all new hires to consult with their healthcare provider regarding the most current vaccination recommendations. Please be aware that, depending on future CDC guidance or regulatory updates, we may require proof of the most recent COVID-19 vaccination as part of our ongoing commitment to health and safety.

Compensation & Benefits

Compensation Range: $45.00 - 48.00 per hour (based on relevant experience)
