Responsibilities
Provide patient‑centered care under the supervision of a Registered Nurse
Complete proper documentation using HomeCare HomeBase EMR
Assist clients with personal care or home management tasks
Assist with basic personal hygiene and grooming, feeding and ambulation, medical monitoring, and other basic health‑care related tasks
Manage basic housekeeping, shopping and related tasks
Qualifications
Minimum age 18 years
Valid driver's license with satisfactory driving record as defined by agency policy
Certification as a CNA or Home Health Care Aide in the state where care will be provided, if required
Ability to use proper body mechanics while lifting; may be expected to lift up to 75 lbs.; able to sit, stand, walk for periods of time and bend, stoop, push, pull, and reach
