Responsibilities
As a Case Manager in the Residential Department, your primary purpose is to coordinate care related to clients assigned in their programs which includes, but is not limited to, discharge planning, CFT coordination, communicating with external stakeholders, and managing other aspects of the client’s care. Your performance will be measured by your individual outcomes, achieving your individual targets/goals, your contribution to your overall program team/department and your buy‑in to the culture of the organization.

Display trauma‑informed practices and principles in interactions with customers, peers and leadership.
Oversee the coordination of successful discharges, referrals, and general needs of the client on your caseload.
Coordinate Child and Family Team meetings, external and internal stakeholder meetings and drive communication around the client’s needs.
Other duties as assigned.
What this position offers
Starting Pay Range: $19.00–$23.00 hourly

Requirements
Must have a valid driver’s license.
Must meet any credentialing, licensing, and privileging standards as required of the job.
Bachelor’s degree in human services or related field.
Minimum of 2 years of relevant professional experience.
Proficient in Microsoft Office Suite application software.
Excellent written and oral communication skills.
Ability to work on‑site at the assigned location during scheduled hours.
Ability to work well with different internal and external stakeholders.
