Responsibilities
The Physical Therapist / PT assesses referred patients requiring rehabilitation services and provides direct patient care, including evaluating medical condition, functional capabilities, limitations, and potential for rehabilitation. The PT establishes and administers treatment programs with specific goals based on the patient’s capacity and tolerance under physician direction, sets realistic goals, documents progress, and educates patients and families on appropriate physical therapy methods.
Develops a plan of care for each patient.
Provides skilled physical therapy services/interventions in accordance with physician orders.
Assesses patient needs, plans, evaluates and modifies care to meet goals.
Collaborates with all disciplines to plan and evaluate team goals for each patient.
Qualifications
Doctorate degree in Physical Therapy OR a Bachelor’s degree in Physical Therapy who has passed the NPTE.
Graduated from an accredited school.
Minimum one year of experience.
Current state license as a Registered Physical Therapist / PT or actively pursuing licensure.
Current CPR certification.
Other requirements determined by client facility.
