Pay Range $24.00 - $27.00 per hour

Position Summary
The role of this Case Manager is to provide case management to participants and to provide advocacy, support, and referral information and assist individuals to overcome barriers to become self-sufficient.

Essential Functions And Responsibilities

Complete intake and needs assessments with each participant, which may involve a broad range of areas including housing, finance & budgeting, employment, education, food & nutrition, school & daycare, legal issues, health, parenting, and daily living skills

Interview and evaluate clients to formulate Individualized Service Plans based on Program requirements. Work closely with clients to identify and build upon strengths and develop strategies to address barriers and concerns identified through the assessment process

Help clients build self‑sufficiency plans. Assist clients to transition to self‑advocacy roles

Advocate on behalf of clients, and attend administrative hearings if necessary

May perform home visits and/or unit inspections

Establish, evaluate and monitor goals for clients linked to measurable outcomes. Work collaboratively with interdisciplinary team of collateral providers and partners to ensure coordination of services

Maintain the structure and policies of the program through orientation of participants by reviewing program rules, policies, and case management visits

Uphold the values of CTI – Equity, Integrity, Compassion

Qualifications

High School Diploma or equivalent education

Prior related experience

Minimum of one year assessment, advocacy and case management work in a Human Service field

Knowledge and experience working with low income, culturally diverse, and at‑risk families and individuals

Demonstrated knowledge of community resources available to low‑income individuals and families

Excellent customer service, organizational, oral and written communication skills

Ability to manage multiple priorities and work in a fast‑paced environment

Proficient computer skills required in basic computer software and data collection systems

Excellent crisis management skills

Ability to maintain accurate records and confidentiality of information

Excellent attention to detail

Valid Driver’s License

Preferred Qualifications

Bilingual (English/Spanish, English/ Haitian‑Creole, English/Khmer, English/Portuguese)
