Position Overview
We are seeking a Field Nurse Case Manager to cover the Atlanta, GA area. This home‑based field position requires travel up to 2 hours one‑way to doctors’ offices, hospitals, and other locations. The Nurse Case Manager is responsible for the medical case management of work‑related injuries, including assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager works with insurance carriers, medical care providers, attorneys, employers, and employees, closely monitors progress, and reports results back to employers and insurance carriers.

Responsibilities

Assess and plan medical care for work‑related injury patients.

Coordinate care with insurance carriers, providers, attorneys, employers, and employees.

Monitor progress of injured workers and report outcomes to stakeholders.

Ensure timely and effective implementation of care plans.

Qualifications

Current Registered Nurse (RN) licence in GA.

CCM, CRRN, COHN, or CDMS required.

GA Rehabilitation Supplier required.

Experience in workers compensation case management preferred.

Strong organization, timeline adherence, PC and technology skills, and written, verbal, and interpersonal communication skills.

Valid driver’s licence and good driving history.
