Compensation:
$52,000.00 annually ($25.00 hourly). Shift differentials apply for evening and night shifts ($0.87 per hour).
Shift Schedule:
12‑hour shifts, 6:30 am to 7:00 pm or 6:30 pm to 7:00 am.

Job Description
Basic Purpose:
Provide direct and indirect non‑professional nursing care or outreach services in clinical, institutional, or home settings under nursing or medical supervision. Responsibilities include taking vital signs, obtaining lab specimens, charting, escorting patients, assisting with activities of daily living, and ordering and receiving supplies.
Typical Functions:
Perform direct nursing tasks such as taking vital signs and assisting with activities of daily living and hygiene needs.
Chart observations and activities, reporting pertinent changes in the patient’s condition.
Perform delegated or specialized functions as educationally prepared.
Escort patients to and from various destinations.
Assist in maintaining a clean, safe environment.
Order, receive, and store supplies and perform basic clerical functions.
Participate in follow‑up activities, track clients, schedule appointments, and perform related tasks.
Provide case finding and recruitment of at‑risk persons for comprehensive health services.
Level Descriptor:
Leader within the job family, responsible for guiding, training, and assigning work activities to other patient care assistants, and performing more advanced duties.
Education and Experience:
Current Long‑Term Care (CNA) certification; Certified Medication Aide certification with the four advancements (Gastro, Respiratory, Glucose, Insulin); three years of experience in Long Term Care.
Knowledge, Skills, Abilities, and Competencies:
Proficient communication in English (oral and written); knowledge of public health services; record keeping; interviewing techniques; basic nursing practices; sanitation and personal hygiene methods; medical terminology; ability to record observations; follow written and oral directions; establish effective working relationships; apply basic nursing techniques; monitor and direct colleagues; accurately assess situations and adopt effective action.
