Responsibilities
The PACE Physical Therapist functions as an integral part of the PACE team and evaluates and treats patients requiring physical therapy services. The Physical Therapist communicates with the PACE Interdisciplinary Team regarding the patient’s treatment plan and progress, and delivers services consistent with PACE program policies and procedures.
Perform examination and evaluation to determine diagnosis and prognosis and establish interventions to achieve patient goals.
Utilize appropriate physical therapy interventions and modalities.
Document progress and status to the PACE interdisciplinary team, including physician, nurses, social workers, occupational therapists, recreation, dietary, home care, etc., to coordinate and manage care of participants at home or in skilled nursing facilities.
Follow through with IDT recommendations and coordinate with vendors for medical equipment.
Maintain professional code of ethics and state licensure requirements.
Supervise a Physical Therapist Assistant in accordance with state licensure laws.
Keep work area and equipment clean and orderly in accordance with safety and infection‑control policies.
Act as a member of the PACE team through joint interventions, communication, quality‑assurance programs, and meetings.
Qualifications
Graduate of an accredited physical therapy program.
Licensed by the state of Iowa and possessing a valid driver’s license with valid auto insurance.
Proficient in using machines (modalities) and administrative tasks such as examination, evaluation, and interventions.
Capable of performing job tasks within an interdisciplinary team environment.
Familiar with and able to use the usual and customary equipment required for essential functions of the position.
Salary includes bonuses, differential pay, or other compensation options as applicable.
