About The Role
Practice nursing the way it was meant to be - with time, autonomy, and the ability to truly impact your patients' lives. As a Home Health Registered Nurse, you'll provide comprehensive skilled nursing care to patients in their homes, helping them recover, manage chronic conditions, and achieve their health goals in the comfort of their own environment. This role is perfect for an RN who values independence, clinical decision-making, and building meaningful relationships with patients and families. You'll manage your own caseload from admission to discharge, collaborating with physicians and a multidisciplinary team while having the autonomy to make clinical judgments that directly shape patient outcomes.

What You'll Do
Provide Comprehensive Nursing Care

Conduct initial evaluations and ongoing OASIS assessments at appropriate time points

Deliver skilled nursing services requiring specialized expertise according to physician orders

Initiate diagnostic, preventive, and rehabilitative nursing procedures

Demonstrate proficiency in phlebotomy, medication administration, and wound care

Monitor and respond to changes in patient conditions promptly

Manage Your Caseload

Assume primary responsibility for assessing, planning, coordinating, implementing, and evaluating care

Develop, implement, and revise individualized plans of care based on patient needs

Make clinical decisions about when to involve other disciplines or escalate concerns

Plan and coordinate patient discharges from services

Maintain accurate documentation within 24 hours of visits

Teach and Empower Patients

Educate patients and families on home nursing procedures and self-care techniques

Teach disease prevention, injury management, and health promotion strategies

Work toward patient and caregiver independence as the ultimate goal

Provide counseling appropriate to each patient\'s condition and learning needs

Use evidence-based teaching materials and techniques

Coordinate Multidisciplinary Care

Collaborate with physicians, therapists, social workers, and other healthcare professionals

Initiate and participate in client care conferences for complex cases

Assign and supervise home health aide services, conducting required supervisory visits

Supervise LPN/LVNs and paraprofessionals providing care

Ensure continuity of care across all providers

Maintain Clinical Excellence

Communicate significant findings, problems, and changes to physicians and supervisors

Report unsafe conditions promptly to appropriate parties

Participate in quality improvement initiatives and in-service programs

Stay current with best practices and regulatory requirements

Follow infection control, safety, and documentation policies

Required Qualifications

Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing

Current State License as a Registered Nurse

Minimum 1 year of experience as a licensed RN in a clinical care setting

Reliable transportation with valid automobile insurance

Willingness to travel daily for home visits within service area

Preferred Qualifications

Bachelor of Science in Nursing (BSN)

Experience in home health environment

OASIS certification or willingness to obtain

Essential Skills and Competencies

Work Environment and Schedule
This position requires daily travel to patient homes and occasional facility visits within your service area. You'll work independently in varied home environments, adapting to different settings and patient situations throughout your day. Schedule changes are common based on patient needs, new admissions, and staffing requirements. Flexibility is essential, and on-call responsibilities may be required. Physical demands include standing, walking, bending, and lifting up to 50 lbs with assistance.

Hours

8:00 am – 5:00 pm, Monday through Friday

Pay Range: $75,000—$85,000 USD
