On‑Call Licensed Practical Nurse – NOC Shift
Bridgeway Community Health is currently seeking a qualified Licensed Practical Nurse to join our growing Medically Managed Withdrawal treatment team.

We are looking for a night‑time professional with strong nursing skills.

The successful candidate will provide exceptional nursing care to our clients, focusing on direct care that promotes health, prevents disease and treats illness.

Primary Purpose
Provides nursing care to individuals in an inpatient medically managed withdrawal setting, supporting health promotion, disease prevention, and treatment of illness. Operates in accordance with the Oregon State Board of Nursing Practice Act.

Essential Duties and Responsibilities

Administers medications and performs treatments and procedures under direction of a nurse practitioner or physician.

Completes accurate and thorough medical documentation, which may include clients' medical charts, medication records, any pertinent observations, computerized dispensing and pharmacy records, and required state or department database entry.

Conducts nursing assessment on all inpatient clients and documents initial and ongoing focused assessments of clients' health status. Reports findings to the Registered Nurse or licensed physician as needed and utilizes appropriate physician standing orders.

Prepares and dispenses or provides for the dispensing of appropriate medication or treatment. Evaluates side effects and potential interactions, adhering to accepted medical practices and medication management per OAR/ORS.

Updates and maintains prescription records for treatment and all related medical records associated with storing and dispensing medications. Complies with federal, state, and program requirements related to record keeping and documentation.

Coordinates client appointments with physician(s).

Educates clients in relation to other disease processes and refers to other medical agencies for follow‑up as needed.

Maintains records of nursing services, including recording statistics and compiling requested reports.

Prepares and labels specimens. Maintains clinic equipment and supplies.

Charts for all clients seen for medical support.

Participates in discharge planning for clients' ongoing medical needs.

Conducts TB skin testing on all inpatient clients who are in treatment for more than 30 days.

Conducts annual TB skin testing on all inpatient clients and staff.

Other Duties and Responsibilities

Attends staff meetings and works as a team member to identify and implement quality of services.

Maintains an organized and clean work environment.

Performs other duties as assigned.

Minimum Qualifications and Experience
Experience, Education and Credentials

Graduation from a state‑accredited School of Nursing program.

Current and unencumbered license to practice as a Licensed Professional Nurse in Oregon.

One (1) year of Licensed Practical Nursing experience highly preferred; new graduates of an accredited nursing program may be considered.

Or any satisfactory equivalent combination of education, training and/or experience relevant to the position.

Knowledge, Skills and Abilities

Comprehensive knowledge of and skill in performing current nursing practice.

Ability to apply nursing knowledge and principles and policies underlying the administration of a nursing service.

Ability to recognize the needs and concerns of individuals during medical treatments and to maintain confidential records.

Ability to accept and abide by supervision received.

Ability to communicate clearly and concisely, orally and in writing.

Skilled in English usage and basic mathematics.

Ability to establish and maintain positive working relationships with co‑workers and the public.

Qualified candidate must be able to pass a criminal background check.

Wage will depend on experience and credentials.
