Schedule: Mon - Fri; 8 am to 5 pm, rotating on call coverage.

Office location: Troy, MI.

Home Care Experience is required.

Overview
As a Clinical Manager, you will make a difference in our patients’ lives by ensuring quality and safe delivery of home health services.

Responsibilities

Operate and ensure start of care compliance, focus on outcome monitors, participate in on‑call rotation, staffing and compliance reporting.

Patient care: Conduct visits as needed and assist with patient issues.

Budget: Manage cost, monitor supplies, overtime, staffing, bonuses and visits per episode.

Hiring & Training: Select and onboard staff.

Collaboration: Work with intake, provide direction on referral volume, maintain relationships with referrers.

Supervision: Oversee liaison and clinical staff, ensure charting, identify education needs, manage on‑call, PTO, call offs, productivity review.

Minimize patient risk: Ensure quality and safe delivery.

Policies: Review documentation, maintain knowledge of Medicare, federal, insurance, company and hospital guidelines.

Qualifications

Current RN or PT license in the state of employment.

Minimum 3 years of RN or PT experience in an acute care setting.

Valid driver’s license and auto insurance.

Capable of all physical demands.
