Responsibilities
Provide occupational therapy services to patients in home health settings in Chippewa Falls, WI and surrounding areas.
Evaluate patient status, develop and implement treatment plans, and adjust interventions as needed.
Collaborate with the care team to ensure quality, patient-centered care and timely OASIS documentation.
Qualifications
Graduate of an accredited Occupational Therapist program.
Licensed as an Occupational Therapist in Wisconsin.
CPR Certified or ability to obtain.
Previous OASIS experience.
Previous experience with computer charting.
6 months of prior Home Health Care experience.
Valid U.S. Driver's License and reliable transportation.
Contract Details
Start Date: ASAP
Duration: 13 weeks
Hours: 40 hours per week
Shift: 8 hours, days
Employment Type: Travel
Pay: $2,000 - $2,166 per week (paid by the hour, not by the visit)
Designation: Occupational Therapist will have a designated territory of coverage
