Qualifications

Required Education: Bachelor's degree in Nursing, Social Work, Healthcare Administration, or a related field.

Required Experience:

Minimum of 3 years of experience in a healthcare or medical setting, preferably in care coordination or case management.

Proven track record of working with diverse patient populations and managing patient care plans.

Experience in collaborating with healthcare professionals to ensure comprehensive patient care.

Required Skills And Abilities:

Strong understanding of healthcare systems, medical terminology, and patient care protocols.
