Registered Nurse License (RN), Licensed Practical Nurse (LPN), or Nurse Practitioner
Minimum of one (1) year experience working with adults with serious mental illness and a working knowledge of psychiatric medications
Licensure / Certification Requirements
RN or Nurse Practitioner
Basic Requirements
Ability to perform tasks on the computer including Microsoft 365, Excel and Word
Valid driver’s license, auto insurance coverage, and reliable transportation
Pass criminal background check and other regulatory checks
Provide proof of your education
Role and Responsibilities
Provide service coordination (case management) for an assigned group of clients, coordinating and monitoring the activities of the individual treatment team
Provide individual supportive therapy and symptom management, ensuring immediate changes in treatment plans as clients need and advocating on behalf of client rights and preferences
Organize and manage the system of getting medication to clients and integrating medication administration tightly into each client’s individual treatment plan
In collaboration with the team psychiatrist, coordinate, schedule and administer medical assessments of client physical health; make appropriate referrals to community physicians for further assessment and treatment, and coordinate psychiatric treatment with medical treatment
