In this skilled rehabilitation setting, you will join a cohesive, interdisciplinary team focused on safe and timely functional recovery for adult and geriatric patients. The facility serves a diverse post‑acute caseload, including patients recovering from orthopedic surgeries, fractures, strokes, deconditioning, cardiopulmonary events, and other complex medical conditions.
Responsibilities
Evaluate new admissions and develop individualized plans of care.
Provide one‑on‑one and small‑group treatments designed to restore mobility, improve balance, increase strength and endurance, and maximize independence in activities of daily living.
Perform gait training, transfer training, therapeutic exercise, neuromuscular re‑education, and patient and family education.
Monitor functional progress and adjust treatment plans as needed.
Collaborate closely with nursing and the broader care team to coordinate safe discharge planning, including recommendations for equipment, home exercise programs, and support services.
Complete documentation in an electronic medical record system with an emphasis on accurate functional reporting and outcome measures.
Adhere to a typical skilled rehab PT schedule, predominantly daytime hours with potential weekend or holiday rotation to support continuous rehabilitation services.
Qualifications
Licensed Physical Therapist, Skilled.
Minimum 1 year of experience in post‑acute or geriatric rehabilitation.
PT(Compact) accepted.
Certification in BLS‑AHA.
