POSITION RESPONSIBILITIES
Demonstrates leadership and provides direction to members of the patient care team through case management of patient home health episode of health care.
Case management of patient services includes:
Make home visits for start of care, recertification
Initial in-home evaluations and/or assessment
Establish the patient’s individualized treatment (plan of care) and medical record baseline
Complete all charting including OASIS and POC
Collaborate with physicians, other agency staff and contract services to coordinate and implement the POC
Document direct patient services, coordination and collaboration with physician and other disciplines or provides in the care of the patient, ensuring document is complete, and complies with acceptable home health standards and Agency policy.
Facilitates active and effective communication with team members and timely clinical decisions which provide guidance in the delivery of patient care.
Documentation within 48 hours and meets professional standards of practice, establishes compliance with state regulations and Medicare conditions of participation.
Actively participates in Agency quality improvement activities
Promotes the Agency and services to the public and participate in physician and community education related to services provided by the agency as registered
Performs other duties as assigned
Maintains responsibility to develop professional self: Attend seminars, and identify self - learning needs, collaborates with agency leaders in meeting learning needs
QUALIFICATIONS
Registered Nurse
Have training and experience in health service administration and at least 1 year in home health care or related health programs
Exhibits leadership skills
Complete a minimum of six clock hours per year of continuing education in subjects related to the duties of the administrator
Be able to read, write and comprehend English
Not been convicted of a felony or misdemeanor
Must be an RN with a public health, home health care, or hospital background
Must have at least one-year clinical experience
Must be able to hear and speak in a manner understood by most persons.
Must be able to travel to prospective patient’s place of residence
Must be able to stoop and bend; must be able to lift and transfer patients

Able to work a minimum of hours per week for successful operations of ADMT work.
Able to bend and stand an average of 6 hours per day.
Lift up to 50 - 75 pounds if necessary.
Able to write up to 3 hours per day.Able to work in a stressful environment.
Able to drive 50-100 miles per day if necessary.
Able to assess and communicate with ill patients, co-workers, and general public.
Is neat in appearance and practice, with good personal hygiene.
ENVIRONMENTAL CHARACTERISTICS
Work environment is generally in a client’s place of residence
In cases of transport, some outdoor may be necessary.
Ability to stand for extended periods
Visual acuity (Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.)
