Role Description
BrightStar Care of Plymouth is seeking a Part‑Time Physical Therapist for an on‑site role based in Minnetonka, MN. The Physical Therapist will evaluate and create customized treatment plans for clients to improve their physical mobility and function. Key responsibilities include providing therapy in home and clinic settings, documenting patient progress, and collaborating with healthcare providers and families to ensure optimal care. The role requires hands‑on engagement, patient‑centered care, and a commitment to maintaining high professional standards.
Qualifications
Proficiency in Physical Therapy, Rehabilitation, and medicine‑related interventions.
Experience in patient care with a strong focus on improving physical mobility and function.
Knowledge of home care practices, including creating treatment plans for in‑home rehabilitation.
Strong interpersonal and communication skills to collaborate effectively with families and care teams.
Bachelor’s or Doctoral degree in Physical Therapy from an accredited program.
Current State of Minnesota Physical Therapist license, or eligibility to obtain one.
CPR certification and familiarity with safety protocols in various care settings.
Previous experience working in home health or outpatient therapy is a plus.
