Job Summary
As a Clinical Rehabilitation Specialist, you’ll guide patients toward recovery by restoring hope, rebuilding strength, and helping them heal. Serving as the inpatient rehabilitation expert between internal and external referral sources and rehabilitation operations, you’ll collaborate with the Rehabilitation Medical Director and care teams to identify candidates for inpatient rehab, perform timely assessments, ensure appropriate placement, and coordinate smooth transfers that connect patients to the right level of care.
Responsibilities
Conducting clinical assessments and screenings to evaluate rehabilitation eligibility and support effective care transitions
Coordinating pre-admission processes by collecting medical, insurance, and resource information to ensure timely, informed admissions
Educating patients, families, and caregivers about program features, recovery goals, and the rehabilitation journey
Building collaborative relationships with physicians, case managers, and referral sources to strengthen communication and alignment
Advancing program growth through outreach, data insights, and community education that support HCA Healthcare’s mission to care for and improve human life
Qualifications
Bachelor’s Degree with a major in Marketing, and/or Licensed Nurse preferred
Minimum of two (2) years of experience in sales or public relations, preferably in a home health care company
Reliable transportation and proof of valid automobile liability insurance
Must have valid driver’s license
Travel required
