Job Summary
The Vice President, Patient Flow & Care Transitions provides executive leadership for systemwide patient flow, capacity management, and care transition operations, ensuring timely access, efficient throughput, and seamless progression of care across the continuum. This role drives enterprise strategies to reduce length of stay, prevent avoidable delays and denials, and optimize transitions through close partnership with clinical, operational, quality, and revenue cycle leaders. The VP oversees integrated care management and patient flow operations, standardizing workflows, strengthening community partnerships, and leveraging data analytics to improve performance, quality, and patient experience. Serving as a member of the executive leadership team, the role ensures regulatory compliance, advances value-based care objectives and builds a high-performing interdisciplinary workforce to meet system capacity and access demands.

Minimum Qualifications
Degrees

Bachelor of Science in Nursing

Master of Science in Nursing, Business Administration, Health Administration or related healthcare field

Licenses & Certifications

Nurse Executive Board Certified (or) Nurse Executive Advanced - Board Certified (NEA-BC) (or) Certified Nurse Manager & Leader (or) Certified in Executive Nursing Practice in Nursing Administration; to be completed within two (2) years;

Accredited Case Manager completed within two (2) years if not held at the time of appointment

Registered Nurse State of Texas or valid multi-state/compact license

Work Experience

Fifteen (15) years work experience in a healthcare environment; specific experience in care or case management including care coordination, utilization review, utilization management, transition planning, and social work and patient flow operations

Management Experience

Ten (10) years of management experience in a management or supervisory role within a complex health care environment including care management

Communication Skills

Above average verbal communication (heavy public contact)

Exceptional verbal public speaking
