$750 - $950 per day
Daily bonus opportunities for optional increased patient load

High 20s per day (Full patient roster consistently available)
Practice Overview
A well-established eye care organization is seeking motivated and personable Optometrists to provide on-site vision care to patients in Skilled Nursing Facilities (SNFs) across Queens, NY. This is a mobile, facility-based role focused on serving the elderly population with full clinical and administrative support.
Optometrists are assigned specific nursing homes and will travel to at least two Skilled Nursing Facilities per day, selected from an assigned group of locations. Providers typically work 5-6 hours per day, finishing once the patient load is completed.
Position Overview
This role is ideal for Optometrists who enjoy autonomy, flexible scheduling, and meaningful work with geriatric patients. The organization handles all scheduling, billing, and administrative tasks, allowing providers to focus entirely on patient care. A medical assistant is assigned daily, and portable equipment is provided.
Key Responsibilities
Provide comprehensive eye exams to nursing home residents
Diagnose and manage vision conditions in geriatric patients
Educate patients and caregivers on eye health and treatment plans
Travel to a minimum of two Skilled Nursing Facilities per day (from assigned locations)
Work independently with clinical confidence and professionalism
Collaborate with assigned medical assistant during patient visits
Maintain accurate and timely clinical documentation

Qualifications
Doctor of Optometry (OD)
Active New York Optometry License (Required)
1+ year of experience preferred (new graduates welcome)
Experience with elderly patients is a plus
Reliable personal vehicle required (travel role)
