Basin Home Health & Hospice is looking for a compassionate
PRN Occupational Therapist
to join our team in Farmington. In this role, you’ll help patients improve independence and safety at home through thoughtful assessment, personalized treatment plans, and practical strategies that support daily living.
This is a great fit for an OT who enjoys meaningful 1:1 patient care, flexibility, and working in a supportive team environment.
What you’ll do:
Evaluate patients' functional ability in the home setting
Develop individualized care plans and therapy goals
Provide therapeutic interventions focused on daily living skills, safety, and independence
Educate patients and caregivers with clear, empowering guidance
Coordinate care with nursing and the broader care team
Supervise Occupational Therapy Assistants and Home Health Aides as needed
What we’re looking for:
Licensed Occupational Therapist (NM license or eligible)
Strong clinical judgment, communication, and documentation skills
Comfortable working independently in the field
Passion for helping people thrive at home
