* Deliver *on-site primary care* to seniors in assisted living, memory care, and independent living
* Provide *hands-on wound assessments and treatment*
* Manage chronic conditions while focusing on prevention and early intervention
* Build strong, consistent relationships with residents, nursing staff, and families
* Communicate clearly with interdisciplinary teams to ensure continuity of care
* Document thoroughly with an emphasis on follow-through and outcomes
