Job Summary
The Community Support Professional (CSP) plays a vital role in helping individuals achieve greater independence and quality of life by providing person-centered support services. This role focuses on assisting clients in navigating various life domains such as housing, healthcare, employment, and personal development, with the goal of integrating fully into the community.

Key Duties and Responsibilities

Establish and maintain professional rapport with clients, upholding strong ethical boundaries

Support client integration into the community by connecting them with local resources for housing, healthcare, and social inclusion

Maintain regular contact with clients as defined in their service plan

Coordinate and assist with referrals for medical care, entitlements, transportation, education, employment, legal assistance, and more

Assist with money management and provide support in obtaining and maintaining entitlements

Develop, implement, and monitor person-centered care plans throughout all stages, including intake and discharge

Enhance independent living skills through direct instruction, modeling, prompting, and hands-on assistance

Identify barriers to clients’ goals and connect them to services that address these challenges

Conduct regular meetings with clients to review progress, mental health stability, appointments, and discharge readiness

Perform routine apartment or room inspections and provide assistance in maintaining cleanliness as needed

Build and maintain collaborative relationships with families, natural supports, and external providers

Facilitate or co-facilitate psychoeducational or skill-building groups as needed

Attend team meetings, staff trainings, supervision, and interdisciplinary rounds

Support clients with completing redetermination forms, SNAP applications, and other essential documents

Complete and submit encounter notes within 24 hours of client interactions

Submit monthly summary reports by the 21st of each month

Qualifications

High school diploma or GED required

Bachelor’s degree in Human Services, Social Work, Psychology, or related field preferred

Minimum of 2 years of experience in homecare or community-based human services

Strong communication, documentation, and organizational skills

Ability to work independently and as part of a team

Valid driver’s license and reliable transportation preferred
