
Case Manager

Coleman Health Services is a nationally recognized not-for-profit provider of behavioral health and rehabilitation services. We are seeking a dedicated and compassionate individual to join our team as an Adult Case Manager in Allen County, OH. In this role, you will be responsible for providing comprehensive case management services to individuals in the community.
Key Responsibilities
Driving to client residences and other locations in the community with client.
Actively engaging with individuals and delivering client-centered care in the community.
Meeting weekly client care hours by spending 80% of your time in the community with clients
Conducting thorough assessments of clients' needs and developing Individualized Service Plans
Coordinating and advocating for needed services, including healthcare, housing, and social support
Collaborating with community resources and providers to ensure clients receive appropriate care
Monitoring and evaluating clients' progress and adjusting Individual Service Plan as needed
Providing support and crisis intervention as necessary
Qualifications
Associates in Human Services, or related field (Bachelor's degree preferred)
Valid Driver's License and maintain excellent driving record.
Ability to obtain NPI Number and eligibility to apply for Ohio Medicaid Number.
