Position Summary
The Case Manager plays a critical role in connecting clients with resources and ensuring they receive comprehensive, trauma-informed support. This position involves assessing client needs, developing individualized care plans, coordinating services, and advocating for clients to achieve stability and wellness.
Key Responsibilities
Conduct thorough assessments to identify client needs and goals.
Develop and implement individualized service plans.
Coordinate referrals to community resources, healthcare providers, and social services.
Monitor client progress and adjust plans as needed.
Maintain accurate and timely documentation in compliance with agency and regulatory standards.
Collaborate with multidisciplinary teams to ensure holistic care.
Advocate for clients to access necessary services and overcome barriers.
Qualifications
Bachelor’s degree in Social Work, Psychology, or related field (Master’s preferred).
Experience in case management or social services strongly preferred.
Knowledge of community resources and trauma-informed practices.
Strong organizational, communication, and problem-solving skills.
Ability to work independently and as part of a team.
