Position: Social Worker
Location: Unity Hospital
Department: Emergency Department
Hours: Full Time – 40 hrs per week
Schedule: Monday–Friday, Days

Responsibilities
Determine patient needs through rounds, chart review, patient/family interviews, and team conferences; coordinate multidisciplinary and agency case conferences; collaborate with care managers to advocate for patient/family and obtain insurance coverage approval.
Manage referrals related to patients at risk, determine appropriate intervention strategies, document implementation, and ensure inappropriate referrals are processed correctly.
Coordinate assessments and develop care plans in accordance with accepted social work policy; implement plans prior to discharge or to initiate continued care.
Perform additional duties as assigned.
Key Responsibilities
Manage referrals for patients with psychosocial needs, review and track census or documentation to identify high‑risk patients not referred, document interventions, and ensure inappropriate referrals are correctly handled.
Assess patient needs, determine mode of intervention, record psychosocial assessments, coordinate assessments and develop care plans, evaluate patients for appropriate level of care, and coordinate case conferences.
Implement intervention plans prior to discharge or for continued care, in compliance with department and regulatory standards.
Work with care managers and health insurance providers to advocate for patient/family coverage, involve patient/family in the planning process, and secure necessary approvals.
Execute discharge or continued care plans, notify involved parties within 24 hours of authorization, request required paperwork from nursing/clinical staff.
Arrange transportation for patients following hospital procedures.
Act as liaison with community and referral source.
Minimum Qualifications
For hires on or before December 31, 2023: Bachelor’s Degree in Social Work or related degree required.
For hires on or after January 1, 2024: Bachelor’s Degree in Social Work required.
New Graduates
Official transcript or registrar confirmation accepted upon graduation.
Primary source education verification required within 90 days of start date.
Education & Licensure
Required: BS in Social Work.
Physical Requirements
Sedentary work; may require walking or standing briefly; may be required to lift up to 10 pounds occasionally.
Pay Range
$24.75 – $34.00
