Internal Admissions Coordinator
Avante at Lake Worth is seeking an Internal Admissions Coordinator to serve as the frontline liaison for patients and families looking for placement within our Skilled Nursing Facility for short‑term rehabilitation, respite, or long‑term care. The Coordinator will interface with Care Navigators and Nurse Liaisons at partner hospitals, communicate via electronic referral systems and telephone, and interact with referral partners such as physicians, hospital case managers, hospice organizations, home‑health agencies, and health‑plan referral departments. The Coordinator will ensure that benefits are acceptable and clearly explained to prospective admissions, representing the facility with area hospitals and other medical professionals.
Qualifications
Effective oral and written communication skills.
Diplomatic interaction with personnel, residents, visitors, and the general public.
Knowledge of medical terminology.
Knowledge of Medicare, Medicaid, and other insurance programs.
Willingness to seek out and incorporate new methods into admission practices.
Proficiency with Outlook, Excel, and Word.
Up‑to‑date understanding of federal and state regulations for skilled nursing facilities.
Ability to work flexible hours, including weekends.
English reading and writing proficiency.
Minimum age of 18 years.
Education and Experience
Bachelor’s degree or equivalent experience in a related field.
3–5 years of experience in long‑term care (e.g., SNF, ALF, hospital, or similar organization) in a comparable position.
Knowledge of federal and state healthcare regulatory standards.
