LPN - Long-Term Care (Part-Time Nights)

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our long-term care team on a part-time night shift schedule. Every other weekend is required. This role is essential in providing high-quality, resident-centered care in a supportive and collaborative environment. The LPN will work closely with RNs and other healthcare staff to ensure the comfort, safety, and well-being of residents.
Key Responsibilities

Provide direct nursing care to residents, including medication administration and treatments
Monitor resident conditions and report changes to the RN or physician
Assist with care planning and documentation
Support residents with daily living needs and ensure comfort measures are met
Maintain accurate and timely medical records
Collaborate with interdisciplinary team members to ensure quality care
Respond to resident needs and emergencies during overnight hours
Active LPN license in the state of New York
Current CPR certification
Experience in long-term care preferred, but not required
Strong communication and organizational skills
Ability to work independently during night shifts
