Responsibilities
Supervise direct care personnel (e.g., Hospital Aide Coordinators, Therapeutic Program Workers) for at least one module, cottage, house, or residential building during an assigned shift.
Serve as a team member for policy development and implementation of work rules.
Ensure compliance with Medicaid standards and labor contract rules.
Resolve labor disputes and matters at the first step level; refer employees to the Employee Assistance Program Coordinator.
Conduct staff training & in-servicing on new policies, procedures, and regulations; monitor implementation of Individual Habilitation Plans for residents.
Complete and sign employee performance evaluations; ensure attendance of direct care staff at team meetings to develop or revise Individual Habilitation Plans.
Maintain adequate staff coverage by scheduling relief, part‑time, and overtime staff; fill out payroll, overtime, and daily employee work schedules.
Review documentation related to resident behavior response and progress as it relates to Individual Habilitation Plans, including 30‑day, 90‑day, and annual reviews, incident reports, and Medicaid surveys.
Review and process requests for clothing purchases and assign to direct care staff.
Review and process staff incident reports and determine if additional action is needed.
Check staff log books for completeness.
Ensure written documentation for assigned residents; verify staff time and attendance.
Review restraint records to ensure proper procedures were followed; complete census reports.
Investigate and report all major incidents.
Ensure staff compliance with treatment habilitation plans.
Relieve supervisors of other units upon request.
Maintain supplies and acquisition of necessary equipment.

Qualifications
24 months of training or experience in providing care to patients and participating in development or carrying out patients' habilitation plans, preferably with developmental or mental disabilities.
Job Skills: Medical.
Supplemental Information
May be exposed to residents who exhibit maladjustment or display violence.
May be required to work weekends, evening shifts, or at off‑ground locations.
Hourly wage will be paid at step 1, unless otherwise specified by collective bargaining agreement or rules outlined in the ORC/OAC.
The final candidate will undergo a criminal background check. Rule 5123‑2‑02 outlines disqualifying offenses that will preclude employment.
Candidate must submit to urinalysis prior to the appointment to test for illegal drug use. A positive result will result in not being offered employment.
