Overview
RN Field Supervisor / Personal Care Services – Brooklyn, NY. Full‑time field‑based position. Salary $115,000–$125,000 yearly, mileage reimbursement. Monday‑Friday 9:00‑5:00 with occasional on‑call. Coverage area: Bronx, Brooklyn, Manhattan, Queens, Staten Island.
Responsibilities
Give and interpret patient information documents (admission packet, plan of care, advance directives, Bill of Rights, consents, patient education).
Develop individualized Plan of Care based on nursing assessment.
Review reports, re‑evaluate needs, supervise all disciplines in the plan of care, and conduct on‑site competency reviews.
Prepare written documentation and summaries of patient status and progress for continuity of home health services; conference with clinical and scheduling staff to assure continuity of care.
Perform patient re‑assessments in the home and discuss any changes in the plan of treatment with the physician/family.
Provide 24‑hour on‑call coverage on a rotation basis.
Qualifications
Current state license as a Registered Nurse.
One year satisfactory nursing experience in Home Care (desired).
Good organizational writing, communication, and interpersonal skills.
Self‑starting, dependable, and adaptable; able to work independently and consistently meet performance standards.
Able to adjust work and communication style based on situational needs.
