Schedule Options
Option 1: PRN Position – Call Outs, PTO, leave – Monday‑Friday, 8:00 am‑5:00 pm, minimum of 2 shifts per month.
Option 2: Full Time – 40 Hours – Monday‑Friday, 8:00 am – 5:00 pm.
Option 3: Part time – 20‑28 Hours per week – Flexible days/hours.
Location
Position includes visits to Greeley and surrounding areas.
Position Summary
This position is responsible for the evaluation, planning, administration, and supervision of Occupational Therapy treatment in accordance with physician referral, state and local standards, regulatory agencies, and departmental policies and procedures.
Core Functions
Evaluate patient’s functional abilities and limitations, determine intervention needs, establish a plan of care, and set functional, measurable goals.
Implement individualized treatment plans, assess ongoing effectiveness, modify plans as needed, involve patient and family in decision‑making, and provide education to interdisciplinary team.
Document according to department and regulatory standards, ensuring legible and timely records.
Communicate patient information and outcomes to physicians, nurses, patients, families, co‑workers, and interdisciplinary team.
Adapt to assignments in caseload, scheduling, and staffing; assist in other patient care areas as needed.
Delegate tasks to COTA, Therapy Technician, and student under department policies, ensuring competency before delegation.
Participate in department and facility process improvements, meet productivity standards.
Minimum Qualifications
• Successfully completed an approved occupational therapy program and hold a current occupational therapy license in the state of practice.
• BLS certification.
• Proficiency with computer software used in an automated patient care environment.
Preferred Qualifications
Additional related education and/or experience is preferred.
