Productivity: 30 points per week. HOME HEALTH OCCUPATIONAL THERAPIST | SUMMARY AND HIGHLIGHTS: The Home Health Occupational Therapist provides comprehensive assessments and therapeutic interventions to help patients regain functional independence within their home environment across El Paso, Teller, and Douglas counties. This role is responsible for developing individualized care plans, supervising assistant staff, and providing critical patient education on safety and medication management while maintaining precise OASIS documentation. This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates HOME HEALTH OCCUPATIONAL THERAPIST | RESPONSIBILITIES: Care Planning: Assess patient care needs and establish mutual goals with patients and caregivers to develop a comprehensive plan of care, including discharge planning. Environmental Assessment: Evaluate the patient’s home environment for safety and identify necessary community resources to ensure an optimum level of care. Clinical Intervention: Implement Occupational Therapy programs based on physician orders, ensuring all interventions meet professional ethics and regulatory standards. Patient Education: Provide vital education to patients and families regarding diagnosis, safety measures, infection control, and comprehensive medication management. Supervision & Collaboration: Supervise Occupational Therapy Assistants (COTAs) and Home Health Aides while coordinating with other disciplines to enhance patient outcomes. Compliance: Complete and submit all required documentation, including OASIS, within established agency guidelines and regulatory timeframes. HOME HEALTH OCCUPATIONAL THERAPIST | REQUIREMENTS: Education: High School Diploma or GED (verifiable) Attendance: Commitment to mandatory attendance during the first 90 days. Licensure: Must hold a current, unrestricted Occupational Therapist (OT) license in the state of Colorado Clinical Skills: Proficiency in performing therapeutic interventions and the ability to recognize and report changes in patient status effectively. Preferred Qualifications Home Health Expertise: Prior experience in a home health setting is strongly preferred. Documentation: Strong proficiency in OASIS documentation accuracy and response.
