A-Line Staffing is seeking a motivated and detail-oriented Home Health LPN This role offers career growth, a supportive work environment, and full-time hours If interested in this Home Health LPN position APPLY NOW for IMMEDIATE consideration Brittney Blackman | bblackman@alinestaffing.com | text 5867107970 HOME HEALTH LPN | DETAILS AND COMPENSATION: Location: Louisville KY 40223 – 100% on-site Covering : (Jefferson, Oldham, Trimble, Shelby, and Henry Counties) Payrate: $37.30/hr + $0.43 per mile Required Availability: Full-Time | Monday – Friday, 8:00 AM – 5:00 PM (No on-call or weekends) productivity benchmarks of 30–32 points per week . 40 hours NOT guaranteed HOME HEALTH LPN | SUMMARY AND HIGHLIGHTS: The Home Health LPN will execute personalized plans of care, monitor vital statistics, and administer treatments while maintaining patient dignity and independence. This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates HOME HEALTH LPN | RESPONSIBILITIES: Clinical Care: Administer medications, feedings, oxygen, and ostomy care as prescribed; monitor and report abnormal vital signs or changes in patient condition. Treatment & Assessment: Perform wound care (including wound vacs), venipuncture, and cardiac assessments. Care Coordination: Collaborate with PT, OT, and Speech Therapists to complete established routines and ensure all physician instructions are met. Documentation: Accurately document nursing actions, patient progress, and outcomes in a timely manner. Compliance: Maintain strict infection control policies and ensure incident reports are completed accurately and promptly. Inventory Management: Order necessary supplies, medications, and feedings to ensure adequate stock for patient needs. HOME HEALTH LPN | REQUIREMENTS: Education: High School Diploma or GED (verifiable) Attendance: Commitment to mandatory attendance during the first 90 days. License: Current LPN License in the state of Kentucky. Skills: Proficiency in wound care, wound vacs, venipuncture, and cardiac assessments. Software: Experience with Homecare Homebase (HCHB) is highly preferred. Travel: Ability to cover Jefferson, Oldham, Trimble, Shelby, and Henry counties (mileage reimbursement provided).
