If interested in this Hospice RN (Home Health) position APPLY NOW for IMMEDIATE consideration Brittney Blackman | bblackman@alinestaffing.com | text 5867107970 HOSPICE RN (HOME HEALTH) | DETAILS AND COMPENSATION: Location: Camp Hill PA 17011 – 100% on-site, home health settings (Primarily covering Cumberland and Perry Counties) Payrate: $56.14/hr + $0.62/mile Required Availability: Full-Time | Monday – Friday, 8:00 AM – 4:30 PM HOSPICE RN (HOME HEALTH) | SUMMARY AND HIGHLIGHTS: The Hospice RN (Home Health) will provide compassionate end-of-life care by performing comprehensive clinical assessments and managing individualized care plans for hospice patients. The RN coordinates with interdisciplinary teams to ensure smooth care transitions while completing essential visits, including admissions, routine care, and death visits. Ultimately, the role serves as a patient advocate, balancing clinical expertise with the management of non-medical support to optimize patient comfort and dignity. This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates HOSPICE RN (HOME HEALTH) | RESPONSIBILITIES: Clinical Visits: Complete admissions, recertifications, routine visits, PRN visits, and death visits. Case Management: Perform comprehensive assessments of high-risk members and collaborate with primary care providers to implement individualized care plans. Interdisciplinary Coordination: Work with physicians, social workers, and discharge planners to transition patients to appropriate levels of care. Patient Advocacy: Address non-medical barriers to care (such as housing and transportation) and protect patient privacy and confidentiality. Documentation: Maintain detailed and accurate records of clinical, functional, and fiscal outcomes using HCHB software. HOSPICE RN (HOME HEALTH) | REQUIREMENTS: Education: High School Diploma or GED (verifiable) Attendance: Commitment to mandatory attendance during the first 90 days Licensure: Active, unrestricted Pennsylvania Registered Nurse (RN) license. Experience: Minimum 1 year of Hospice experience is required. Software: Proficiency with Homecare Homebase (HCHB) is required. Credentials: Valid BLS/CPR certification, Driver’s License, and active Car Insurance. Productivity: Ability to meet a benchmark of 5 to 7 visits per day. Preferred Qualifications Experience with OASIS Case Management certification. Associate’s or Bachelor’s Degree in Nursing. Take the Next Step!
