Job Overview
The Patient Flow Management Center (PFMC) and Center for Operational Resource Efficiency (C.O.R.E.) collaborate with patient care areas, support services, and administration to implement strategies that ensure seamless patient care, capacity management, and efficient patient flow. Goals include reducing missed transfer opportunities, increasing targeted volumes, decreasing unnecessary length of stay (LOS), and placing patients in the appropriate setting at the right time. The department defines analytics to predict and drive real‑time decisions, tracks metrics, and reports outcomes to sustain continuous improvement.

Job Description
Primary Function

– The Patient Flow Coordinator supports the daily operations of the PFMC. The Coordinator orients, coaches, evaluates, and schedules other Coordinators to ensure timely patient placement across the Division 24 hours per day, 7 days per week. They monitor operational tiles to expedite and prioritize challenges related to high occupancy and resource over‑utilization, coordinate and communicate with key departments (Medical Staff, Nursing, Ancillary Departments, Environmental Services, Patient Transportation, and the Jefferson Transfer Center), and participate in bed coordination to maximize bed utilization.

Essential Functions

Expedite and prioritize administrative issues in real time to remove barriers to discharge and address care delays.

Maintain patient outcomes and continually evaluate bed placement progress according to the Patient Flow Plan.

Coordinate direct admissions—urgent and scheduled—and contact patients and families when beds are available.

Orient, coach, evaluate, and schedule Patient Flow Coordinators.

Participate in projects, data collection, and other activities designed to enhance bed operations.

Identify and communicate quality and risk issues to appropriate staff and departments promptly.

Monitor the EVS and Patient Transport Command Center to ensure bed cleans and transports align with bed prioritization.

Provide leadership for Patient Flow Coordinators and collaborate with PFMC colleagues to address flow issues.

Assist PFMC leadership in defining administrative triggers and feedback mechanisms to inform operational teams, physician leaders, and administrators, mitigating over‑capacity situations.

Support PFMC leadership in developing surge‑management strategies for rising patient volumes.

Specific Actions / Decisions

Assign beds to patients from all clinical departments, direct admissions, and external transfers based on the defined workflow.

Liaise with PFMC staff and providers regarding administrative priorities and synchronize activities based on system needs.

Interact with co‑workers, visitors, and other staff consistently with Jefferson values.

Educational / Training Requirements

High School education – Required

Associate’s or Bachelor’s degree – Preferred

Experience Requirements
1–2 years of healthcare/medical admissions experience – Required

Additional Information

Proficiency with MS Office (Outlook, Word, Excel)

EMR skills and knowledge – Preferred

Customer service, critical thinking, problem‑solving, relationship building, interpersonal skills

Salary
$17.00 to $23.64 per hour (based on experience, skills, and qualifications). This position is not eligible for an annual incentive.
