LVN, PRN, Sunday 10pm-6am
About the Role
The Licensed Vocational Nurse (LVN) delivers high‑quality nursing care to residents within the community, ensuring services are provided in accordance with established clinical standards and practices. Working under the supervision of a licensed nurse or healthcare manager, the LVN implements resident care plans safely and effectively, supports residents’ overall well‑being, and contributes to a positive, safe, and supportive living environment.
Responsibilities
Resident Assessment: Evaluate residents’ physical, psychological, environmental, and social needs, identifying potential health concerns.
Care Delivery: Assist residents with activities of daily living (ADLs), personal hygiene, and mobility while ensuring safe and high‑quality care.
Health Monitoring: Collect and analyze health information, maintaining accurate and up‑to‑date resident records.
Treatment Implementation: Administer medications and treatments per physician orders, monitor for adverse reactions, and report concerns promptly.
Collaboration: Communicate with care teams, physicians, and supervisors regarding changes in condition, hospitalizations, discharges, and appointments.
Care Planning: Implement individualized service and care plans as directed by the Director of Health Services.
Infection Control: Follow universal precautions and infection control protocols in all aspects of care delivery.
Emergency Response: Recognize and respond to emergencies or life‑threatening situations, providing first aid and initiating appropriate actions.
Resident Education: Support residents in developing self‑care skills and understanding health practices.
Team Support: Participate in team meetings, collaborate with nursing staff, and attend in‑service training programs.
Documentation: Maintain accurate, timely documentation in accordance with service and care plan requirements.
Additional Duties: Perform other assigned tasks while adhering to all company policies and procedures.
Qualifications
Education: Graduate of an accredited LVN or LPN program.
Licensure: Current Licensed Vocational Nurse (LVN) license in the state of Texas.
Experience: Minimum of one (1) year of nursing experience in a healthcare setting; experience in geriatric nursing, emergency care, and first aid preferred.
Continuing Education: Must maintain required continuing education credits in accordance with state regulations.
Specialized Training: Experience or training in Alzheimer’s and dementia care, including Validation Therapy, preferred.
