Job Summary
The salary range for this position is $40.79 – $55.07 hourly. Placement depends on relevant work experience, internal equity, and the collective bargaining agreement for union positions.
This is a part‑time position, 20 hours per week, at our Lake Forest Outpatient Clinic.
Clinic hours are 7 am–7 pm Monday–Thursday and 7 am–5 pm Friday.

Position Overview
The Physical Therapist reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and regulatory and accreditation standards.
The position includes direct patient care, teaching, participation in appropriate committees and departmental process improvement, and serves as a role model for all therapists and clinical interns in professional, clinical, communication, education, and leadership activities.

Provide accurate information and timely updates to patients and customers, address questions promptly or identify the appropriate person and resources.

Maintain primary responsibility over patient plan of care through timely collaboration with treating PT/PA/TA.
Demonstrate teamwork by helping co‑workers and colleagues.
Communicate effectively and respect diverse opinions and styles.
Use organizational and unit resources efficiently.
Manage work schedule efficiently, completing tasks on time.
Contribute to continuous improvement opportunities.
Participate in efforts to reduce costs, streamline work processes, and improve services.

Show essential skill competency in selecting evaluation, treatment modalities, equipment, and protocols.
Perform re‑evaluations according to department guidelines and update treatment plan, documented in EMR.
Adhere to department policy regarding evaluations, progress notes, and discharge summaries.
Complete daily notes reflecting patient response, performance, and assessment.
Provide educational materials and methods appropriate to the patient’s level of understanding.
Participate in committees, department processes, and special projects per clinical expertise.
Work to develop professional relationships with healthcare teams.
Engage in internal and external marketing per department expectations.
Create a respectful communication culture by listening, asking for input, and acknowledging contributions.
Accept constructive feedback and make timely changes.
Proactively complete non‑patient care responsibilities within timelines.
Contribute to educational efforts such as in‑services, journal clubs, new hire orientation, and PI.
Actively participate in student clinical or observation programs as an instructor, mentor, or host.
Maintain productivity standards per department expectations.
Demonstrate willingness to flex schedule or assignment to meet patient care needs and additional meetings.
Qualifications
Required
Bachelor’s Degree in Physical Therapy.
Licensed Physical Therapist in the State of Illinois.
BLS certification from the American Heart Association upon hire.
Preferred
Master’s or Doctoral Degree in Physical Therapy.
