Physical Therapist I

Summary
This position delegates responsibility to the Physical Therapist Assistant and Aides. Activities are patient‑focused and involve assessment, planning, intervention, and evaluation in line with physical therapy standards, physician orders, and governing policies and procedures.
Responsibilities
Evaluates the patient’s function by performing formal/informal testing measures to determine skeletal, neurological, respiratory, and cardiovascular status.
Develops a goal‑specific treatment regime based on assessment data and the patient’s tolerance, and carries out the treatment plan as prescribed.
Submits a written treatment regime for inclusion in the plan of treatment approved by and coordinated with the physician, and updates the plan of care as needed.
Creates a treatment schedule that is frequency and duration specific, discussed with patient/family/caregiver, and coordinated with clinical support staff.
Educates patient, family, and/or caregiver about pathology, alternatives, prognosis, and rationale for treatment using rest, exercise, modalities, and other interventions.
Other Duties and Responsibilities
Provides supervision for Physical Therapist Assistants, Aides, Home Health Aides, and students per affiliate and regulatory standards.
Recommends adaptive/supportive equipment based on assessment and assists in securing it for the patient.
Documents patient/family/caregiver responses to treatment and changes in patient condition per affiliate policies.
Completes daily documentation necessary for billing procedures per affiliate policy.
Provides treatment intervention and demonstrates competence to facilitate optimal functioning.
Educates patient/family/caregiver in home education program using effective teaching methods.
Demonstrates effective oral and written communication with all internal and external customers.
Promotes education and development among staff, students, and other health care providers.
Promotes process improvement and assists with program development.
Participates in team conferences and interdisciplinary meetings as appropriate.
Promotes physical therapy within MyMichigan Health and the service area, including community education.
Functions as case manager for assigned cases.
Demonstrates commitment to MyMichigan Health and its affiliate mission through customer service, quality of service, and teamwork.
Maintains confidentiality of patient and affiliate information in accordance with policies and procedures.
Certifications and Licensures
Physical Therapist license (PT)
Basic Life Support (BLS)
Equivalent Experience: Within 1 month of hire or transfer
