Position Summary
This position assesses, plans, implements, evaluates, and documents nursing care of patients in accordance with organizational policies and standards of professional nursing practice. The nurse is accountable for the quality of nursing services delivered by self or by staff under their direction.
Core Functions
Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs; reviews patient history; collaborates with other health care team members; evaluates and reports abnormal lab results to the physician(s); completes assessments and reassessments as outlined by policy.
Formulates and updates a plan of care, including discharge plan, based on assessment data, patient/family input, and the medical plan; develops realistic, measurable goals in collaboration with patient/family; incorporates cultural, psychosocial, and educational needs; revises the plan as patient needs change.
Implements the plan of care through direct patient care, coordination, delegation, and supervision of health care team activities; ensures compliance with policies, procedures, standards of care, and regulatory requirements; provides nursing supervision to other licensed nurses and personnel; communicates patient status accurately to caregivers.
Evaluates patient and family response to care and teaching; revises the plan of care as needed; evaluates patient progress toward goals and expected outcomes in collaboration with the health care team; assesses effectiveness of patient teaching.
Documents assessment, planning, implementation, and evaluation in the patient record; maintains legible, timely documentation that reflects objective/subjective data, interventions, and patient responses; accurately records physician orders.
Provides care based on best evidence; may participate in research activities; engages in unit leadership; educates, mentors, and orients facility personnel, patients, students, families, and visitors; supports growth through precepting when appropriate.
Contributes to society through activities that improve patient outcomes, such as participation in nursing quality indicators and professional organizations.
May supervise staff and workflow of the department (in some roles).
Minimum Qualifications
Must possess a current, valid RN license in the state of practice, a temporary/interim RN license, or compact RN licensure for the state of practice. BLS certification is required. The nurse must maintain clinical performance competencies with annual validations, and must have at least one year of acute‑care experience within the past 12 months. Additional certifications or continuing education may be required based on the area of practice.
Preferred Qualifications
Bachelor's degree preferred. Professional certification preferred. Additional related education and/or experience preferred.
