Duties and Responsibilities
Assessment of referred patients that require rehabilitation services.
Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
Establishing and administering a treatment program with specific goals determined according to the patients' capacity and tolerance under the direction of a physician.
Setting realistic and achievable goals for their patients.
Documenting and recording the patients’ condition.
Educating patients and families in an appropriate therapy method.
Evaluations, weekly progress notes, recertification and discharge documentation.
Collaborating with all disciplines to provide best patient-centered treatment and care.
Performing all other duties assigned by Director of Rehabilitation.
Compliance with confidentiality, HIPAA and Federal Resident Rights.
Qualifications and Requirements
Must have graduated from an accredited Speech Therapist college or university.
Must have a valid professional license and registration for the state of PA.
Experience with long‑term care and/or geriatric care is preferred but not required.
