Hospice Social Worker - Home Care & End-of-Life Support

Hospicio Toque de Amor, located in Manatí, Puerto Rico, is seeking a full-time Social Worker to provide comprehensive care to terminally ill patients and their families. The ideal candidate will assess psychosocial needs, develop tailored care plans, and advocate for patients within an interdisciplinary team. Required qualifications include a Master’s degree in Social Work, current licensure in Puerto Rico, and strong communication skills in English and Spanish. Experience in hospice or healthcare settings is preferred.
