Role Description
This is a full-time on-site role for a Social Worker, located in Manatí, Puerto Rico. The Social Worker will support patients and their families by assessing psychosocial needs, providing counseling, and connecting them with community resources. Responsibilities include developing care plans, advocating for patients, and collaborating with interdisciplinary teams to ensure holistic care. The role involves actively participating in crisis intervention and end-of-life planning.
Qualifications
Strong assessment and counseling skills, with experience in providing emotional support to individuals and families
Knowledge of community resources and expertise in advocacy and service coordination
Skills in end-of-life planning, crisis management, and developing personalized care plans
Proficiency in verbal and written communication in both English and Spanish
Ability to work collaboratively within interdisciplinary teams
Master's degree in Social Work from an accredited institution
Current social work licensure in Puerto Rico
Prior experience in hospice or healthcare settings is strongly preferred
