
Care Coordinator

Primary job responsibilities involve service linkage and care coordination toassistconsumers in addressing their behavioral health needs and tracking relevant health outcomes. Case Managers will provide community support, case management and service linkage both within Boundless and with outside providers thatidentifyand address behavioral health needs, developmentalissuesand barriers to care.The CareCoordinatorwill work with consumers, parents/guardians, Boundlessstaffand outside providers that support treatment and contribute meaningfully towholehealth outcome of the individual served.
Essential Functions:

Providescase management support to consumers engaged in behavioral health services including community psychiatric support and therapeutic support to promote the coordination and implementation of treatment goals
Serve as a liaison between families and agency to positively promote collaborative processes in the treatment planning for clients by encouraging parent/guardian participation in treatment.
Identifyand assesspsychosocial and life skills needs through analysis of factors impinging on clientsuccessincluding factors in the home,schooland community.
Provideparent/guardian educational workshops onidentifiedissues related to child development, stress reduction, discipline and safety, and teacher/parent/client communication.
Keep caseload spreadsheet (information management) updated regularly.
Manage phone calls from families, guardians, agencies, etc. Set up meetings; obtain information, etc.
Providescase management (coordinate and link all services) for clients and families.
Develop programs to address parent/guardian participation in agency servicesand client engagement in thetreatmentprocess.
Maintain all required clinical records andsubmitsappropriate documentsfor statistical reports with adherence to program standards.
Locate and mobilizecommunity resources to support the treatment.
Attend meetings and professional development activities asrequired.
Provide22hours of billable service each week.
All other assigned duties.

Position Type/Expected Hours of Work:
This is a full-time position, and hours of work and days are Monday through Friday, 8:00 a.m. to 4:30 p.m. Occasional evening and weekend work may berequiredas job duties demand.
Travel:
Travel is primarily local during the business day, although some out-of-the-area and overnight travel may be expected on occasion.
Education/Experience:
Bachelor’s degree in social work, education, psychology, or another related field is preferred
High School and GEDrequired
Two plus years working in Human Servicesrequired
Two plus years of experienceprovidingcase management services preferred
Experience working in the behavioral health field preferred.
